Insurance and Facility Use Agreement

A Facilities Use Agreement must be signed and returned to us prior to your wedding.  There is a link to the agreement at the bottom of this page in PDF Form format.  Fill out the form and sign electronically or fill out, print and sign.  You can then either mail the signed form to the P.O. Box below or scan the form and email it to:

The agreement states that the “Renter must provide a Certificate of Insurance naming St. Elizabeth Parish as a co-insured. (St. Colman’s Church is a mission church of St. Elizabeth’s Parish in Guerneville.) For more insurance information, click here.

The Insurance Certificate should be made for $1 million and must be received no later than 30 days prior to the event. 

The certificate will usually be in the form of a rider to your homeowner or renter insurance that adds St. Elizabeth’s Parish as an insured party for the day of the rehearsal and the day of your wedding.

Email or Snail-Mail a copy to us.  Your insurance agent will know what to do.  The certificate, the rental fee and the security deposit should be mailed to:
